| Do applicants need to provide evidence of their disability? |

Applicants may need to provide evidence of their disability if the assessor if unfamiliar with the applicant’s medical history. In such cases, the assessor will seek the applicant’s permission to gather such further information about his/her disability as may be required to carry out a proper assessment. The applicant may therefore be required to provide the assessor with the name of his/her caregiver, ACC assessor, GP, specialist, physiotherapist, psychiatrist, social worker, occupational therapist etc.
| How does temporary registration differ from permanent registration? |

The applicant will be assessed as having either a permanent or a temporary disability. Applicants who meet the eligibility criteria and are assessed as having a permanent disability, will be registered as a ‘life-member’ of the scheme and issued with a photo ID card that only has to be renewed every five years.
Applicants who are assessed as having a temporary disabilitydisability that has lasted or is likely to last for six months or less — will be registered as temporary members, advised when they are due for reassessment, and issued with a temporary photo ID card. A reminder about the need to be reassessed will be sent out about a month before the due date. If the temporary member continues to meet the eligibility criteria he or she will be issued with a replacement card. This process may be repeated as often as necessary.
| Are applicants eligible for Total Mobility if they are receiving transport-related financial assistance from another official source? |

Total Mobility vouchers may only be used for taxi journeys that are not covered by travel income or assistance from another official source. Using financial assistance in conjunction with vouchers (‘double dipping’) is not allowed.
Applicants who receive travel income or assistance must indicate whether they receive general financial assistance or alternatively what journeys they receive financial assistance for. Applicants that receive general financial assistance may not be eligible for Total Mobility at all.
